Practitioner code is required to purchase one of our products. You may get a
practitioner code from one of your nearest
Metaformula corp hospitals
Click 'Register' button on the top right hand side of MetaFormula website.
Choose Practitioner Account and fill out application form.
Application can be accepted only when a practitioner code is valid.
In order to get a practitioner code, please ask one of medical institutions in which
you have ever visited.
Once your application has been accepted, you may purchase our products from Professional Products category.
